Formularium gratiarum . The cover bears an incorrect characterisation of contents, saec. XIX: 'Le sei Decretali Benedettine = Inedite. Rarissime, e vanno unite alle Decretali Innocenziane e Clementine. Valutate dieci Louigi'. In fact this is not a collection of decretals. It is just a collection of copies of successful requests to obtain a favour ('gratia') from the pope - meant to serve as models for future requests of the kind. Probably collected and used as models by some procurator at the Roman Curia who drafted requests of the kind against payment from the interested party
